Marburg Virus History: Shocking Timeline of Outbreaks and Global Impact

Introduction

Marburg virus disease (MVD) is caused by Marburg virus, a member of the Filoviridae family. It is characterized by high fever, systemic illness, bleeding, manifestations, and multi-organ complications. Marburg virus disease has been one of the most severe viral haemorrhagic fevers affecting humans, since its discovery in 1967.

Unlike some emerging infectious diseases that spread widely across continents, Marburg virus outbreaks have generally remained geographically limited within Africa. However, the case fatality rates have been high, particularly when detection and infection-control measures are delayed.

Discovery of Marburg Virus: 1967

In 1967, outbreaks of unknown haemorrhagic fever occurred simultaneously in Marburg and Frankfurt, Germany, and Belgrade, Yugoslavia, marking the beginning of Marburg virus.

The outbreaks occurred among laboratory workers who had been exposed to tissues from imported African green monkeys used for research purposes. It represented the first recognized human infection caused by a filovirus.

The 1967 outbreak resulted in 7 deaths out of 31 cases in Germany and Yugoslavia.

First African Marburg Virus Outbreak: 1975

The first documented African-associated Marburg virus outbreak occurred in 1975 in South Africa. A traveller returning from Zimbabwe developed illness and subsequently transmitted the infection to close contacts.

Although the outbreak was small, with just three cases and one casualty, it confirmed that Marburg virus was not limited to laboratory-associated events and could emerge naturally.

Early Sporadic Marburg Cases in Kenya: 1980-1987

During the 1980s, early Marburg virus infections occurred in Kenya through a traveller who developed the disease after visiting regions associated with potential animal exposure.

Similarly, in 1987 another Marburg virus infection case occurred resulting in death.

Democratic Republic of Congo Outbreak: 1998-2000

The outbreak in Durba, Democratic Republic of Congo (DRC) is one of the largest outbreaks in Marburg virus history.

Most of the cases occurred among miners in a gold-mining environment, where bats or other wildlife were present. Out of 154 cases from 1998 to 2000, there were 128 deaths.

This huge outbreak highlighted several major challenges:

  • Difficulty in recognizing Marburg virus
  • Risk of healthcare-associated transmission
  • Importance of laboratory confirmation
  • Need for rapid infection-control measures

Angola Outbreak – The Largest Marburg Epidemic: 2004-2005

Between 2004-2005, the world observed the most devastating Marburg virus outbreak in Angola.

The outbreak was concentrated mainly in Uige Province and resulted in 329 deaths out of 374 cases. Healthcare settings played an important role in transmission, with infections occurring among patients, caregivers, and healthcare workers.

Repeated Marburg Virus Outbreaks in Uganda: 2007-2017

Uganda became one of the countries most frequently associated with Marburg virus outbreaks and isolated cases.

Several outbreaks occurred between 2007 and 2017, demonstrating continued viral circulation in environments where humans could encounter infected animal reservoirs. In 2007, 4 cases were seen with 2 deaths. Similary, the 2012 and 2017 outbreak resulted in 4 and 3 deaths respectively.

Recent Outbreaks Reported by WHO and CDC: 2021-2024

In August 2021, Guinea reported the first confirmed Marburg virus disease case in West Africa.

According to outbreak records, the case involved a single patient who developed severe illness and died. The event was important because it demonstrated that Marburg virus activity was not limited to its previously recognized areas of circulation in Central and East Africa.

In 2022, Ghana reported its first Marburg virus disease outbreak. The outbreak involved three confirmed cases, including two deaths.

The year 2023 represented a major period in Marburg virus history, with outbreaks reported for the first time in both Equatorial Guinea and Tanzania.

Equatorial Guinea declared its first Marburg virus disease outbreak in February 2023. According to CDC outbreak data, there were 16 confirmed cases and 12 deaths. Furthermore, 23 probable cases were detected, all resulting in death.

Tanzania reported its first Marburg virus outbreak in the Kagera region in March 2023. The outbreak was declared over after several weeks of response activities. The Tanzania outbreak resulted in 6 deaths with 67% case fatality rate.

On 27 September, 2024, Rwanda reported its first Marburg virus disease outbreak. The outbreak was notable because a large proportion of infections occurred among healthcare workers, particularly those working in healthcare facilities in Kigali.

Compared with earlier outbreaks such as Angola (2004–2005), Rwanda recorded a much lower fatality rate of 23% and 15 deaths. This was possible due to:

  • Early detection
  • Laboratory testing
  • Isolation systems
  • Supportive clinical management
  • Infection prevention measures

Why Marburg Virus Remains a Global Health Concern

Although Marburg virus outbreaks are uncommon, the virus remains an important public health threat because of:

  • High mortality potential
  • Rapid progression of disease
  • Difficulty in early recognition
  • Risk of healthcare transmission
  • Potential for emergence in new geographic regions

The history of Marburg virus demonstrates a repeated pattern: introduction from animal reservoirs, human transmission, and severe outbreaks when detection and response are delayed.
